We will closely monitor and address your health concerns with the assistance of our nurses four times a week, along with the coordination of your physician and other healthcare providers. You will receive an Individual Service Plan (ISP) based on your interests, needs, and preferences to promote your optimal health.
Our nurses can help to:
- Administer resident assessments
- Create service plans for each resident
- Develop meaningful interventions
- Evaluate the effectiveness of each resident’s care plan.
- Monitor the status of chronic illnesses
- Manage medications
- Coordinate with external providers (such as home healthcare, hospice, and therapy providers, and physicians)
- Manage disease, chronic illnesses, and other conditions
